My Appointment
If you are a new patient, we have all the information you need to prepare for your appointment, including our New Patient Form that you can fill out in advance. If you have further questions, please feel free to give us a call at 541.506.6500.
Insurance and Billing
If you have health insurance, we will need a copy of your identification card. Our office will call your insurance company to verify your eligibility and benefit coverage and determine whether prior authorization and/or pre-certification is required for hospitalization or surgery.

Travel Information
MCMC Orthopedic Institute is located in the beautiful Columbia River Gorge. We have offices in Hood River and The Dalles, Oregon. Hood River is a scenic, 58-mile drive from Portland International Airport. The Dalles is 22 miles further from PDX. There are many lodging options in our region.
